We are thrilled to announce that Lakeview Family Health Team has received a generous $5,000 donation from the Brian Todd Memorial Community Fund in June 2024. This donation has been instrumental in supporting our HealthInfo Connect Initiative, which includes creating a new website with health information for patients and community members, as well as launching an email newsletter to share health information with the residents of the communities we serve.

Introducing the HealthInfo Connect Initiative
Our HealthInfo Connect Initiative is dedicated to improving access to crucial health information and resources for the residents of the communities we serve, including Brighton. This initiative aims to empower individuals to make informed healthcare decisions and enhance their overall well-being.
Addressing the Healthcare Gap
With the recent departure of key physicians, the issue of residents lacking primary care providers has become more pressing. This affects thousands of individuals in the communities we serve. By offering comprehensive resources on alternative healthcare options, our initiative seeks to bridge this gap and promote various benefits.
Benefits of the HealthInfo Connect Initiative
Even if residents continue to lack access to primary care, access to information about local healthcare services, healthy living, and preventive health practices can provide several benefits, including:
- Awareness of Available Resources: Knowing what services are available can help residents navigate their healthcare options more effectively.
- Health Education: Educating the community on various health topics can lead to better health outcomes.
- Improved Health Literacy: Increased understanding of health information enables residents to make informed decisions about their health.
- Overall Well-Being: Access to health information contributes to the overall well-being of the community.

Our HealthInfo Connect Initiative aligns with the goals of the Brian Todd Memorial Community Fund, which focuses on improving health and wellness for Brighton residents and enhancing community assets. By providing comprehensive resources on alternative healthcare, promoting preventive health practices, and increasing health literacy, we aim to improve overall community health.
