About Lakeview Family Health Team

Formed in 2006, the Lakeview Family Health Team is a team of health care professionals supporting the patients of the Brighton-Quinte West Family Health Organization physicians and people in the communities of the Town of Brighton, Township of Cramahe, and City of Quinte West.

In addition to traditional healthcare services, family health teams focus on health promotion, illness prevention, and chronic disease management.

Our Mission:

The Lakeview Family Health Team will continue its leadership role in providing comprehensive health care services. Through quality improvement, good governance and community engagement, we will support mental and physical wellbeing.
