The Lakeview Family Health Team (LVFHT) is a multidisciplinary team of professionals working in conjunction with patients to identify the needs and strengths within the community. These needs will be addressed through a holistic approach. We will promote and maintain good health and manage chronic disease through preventative care practices for every age group.
Mission
With an understanding that the Mission Statement of the Lakeview Family Health Team explains what the organization must do day-to-day to make the Vision statement a reality, the following Mission was thoughtfully considered and felt to embody the purpose of the organization:
