Social Worker

Job description

The role works to full scope of practice in an interdisciplinary team model of primary care to:

  • Assist in the assessment and management of patients with mental and psychosocial concerns
  • Monitors patient goals and outcomes through ongoing assessment and management including psychotherapy, case management, system navigation and referral
  • Assess patient health care needs and respond by making clinical decisions and exercise judgment about patient care and treatment plan.
  • Maintain accurate and concise and confidential personal health information for patients within the TELUS Practice Solutions (Electronic Medical Record) for documentation.
  • Maximize patient preventative health care activity and optimal health through recommendations on treatment planning in partnership with the Primary Care provider of the Family Health Team and Physician group.

Qualifications

  • Bachelor Degree in Social Work or Social Science
  • Strong patient care and case management skills
  • Experience working in community setting in an interdisciplinary team
  • Knowledge of community resources and strong relationship with community stakeholders.
  • Relevant certifications in social work or related fields

Job Types: Part-time, Permanent

Expected hours: 32 per week
