Job description
The role works to full scope of practice in an interdisciplinary team model of primary care to:
- Assist in the assessment and management of patients with mental and psychosocial concerns
- Monitors patient goals and outcomes through ongoing assessment and management including psychotherapy, case management, system navigation and referral
- Assess patient health care needs and respond by making clinical decisions and exercise judgment about patient care and treatment plan.
- Maintain accurate and concise and confidential personal health information for patients within the TELUS Practice Solutions (Electronic Medical Record) for documentation.
- Maximize patient preventative health care activity and optimal health through recommendations on treatment planning in partnership with the Primary Care provider of the Family Health Team and Physician group.
Qualifications
- Bachelor Degree in Social Work or Social Science
- Strong patient care and case management skills
- Experience working in community setting in an interdisciplinary team
- Knowledge of community resources and strong relationship with community stakeholders.
- Relevant certifications in social work or related fields
