Our Lung Health Program is dedicated to improving the life quality of individuals diagnosed with Chronic Obstructive Pulmonary Disease (COPD) and Asthma. We offer a comprehensive, evidence-based approach, adhering to the best practice guidelines to manage symptoms, slow disease progression, and help patients maintain an active life.
What We Offer:
Team-Based Care: Our interdisciplinary team includes a Certified Respiratory Educator, nurses, and a dietitian, all working together to provide personalized care, including spirometry tests to diagnose and monitor lung health accurately.
Education and Self-Management: We equip our patients with the necessary education to manage their condition effectively. This includes sessions focusing on lifestyle strategies, medication management, and coping mechanisms to handle symptoms and prevent complications.
Continuous Support and Monitoring: Our program offers regular follow-ups to adapt and optimize each patient’s care plan. By staying proactive, we aim to enhance your overall well-being and prevent any lung-related complications.
Eligibility and Joining the Program:
This program is available exclusively to rostered patients of physicians at the Brighton Quinte West Family Health Organization. To join, you will need a referral from your family physician. Following a consultation, eligible patients will be referred to our Lung Health Team for comprehensive management.
