Diabetes Health Program

Our Diabetes Health Program is specifically tailored for individuals aged 18 and over who are diagnosed with Type I, Type II, or pre-diabetes. This program is designed to empower you with the necessary knowledge and skills to effectively manage your diabetes and improve your overall quality of life.

What We Offer:

Personalized Care: Partner with a dedicated care team—including your primary care provider, a certified diabetes educator, a dietitian, and other health specialists—to develop a personalized care plan. Whether you are newly diagnosed or have been managing diabetes for years, we provide the support you need.

Educational Workshops and Individual Consultations: Gain valuable insights into nutrition, medication management, and lifestyle adjustments through our educational workshops or personalized one-on-one consultations. Our programs are designed to equip you with practical tools for daily diabetes management.

Ongoing Support and Monitoring: Regular follow-ups with our team ensure that you remain on track and any adjustments needed in your care plan are addressed promptly. Our objective is to assist you in maintaining control of your diabetes, thereby minimizing the risk of complications. Those achieving control of their diabetes will be gradually transitioned back to their Primary Care Provider for ongoing maintenance, which may include semi-annual or annual check-ins.

Eligibility and Joining the Program: This program is available exclusively to rostered patients of physicians at the Brighton Quinte West Family Health Organization. To join, you will need a referral from your family physician. Following a consultation, eligible patients will be referred to our Diabetes Health Team for comprehensive management.
