Hypertension Program

Our Hypertension Program is designed to manage and improve the health of patients with hypertension, following the Canadian Hypertension Education Program (CHEP) guidelines. This program focuses on monitoring blood pressure, educating patients on lifestyle changes, and providing tailored care to manage hypertension effectively.

What We Offer:

Personalized Management: Led by our care team including a Registered Nurse and Registered Dietitian, we provide individual consultations to monitor and manage hypertension. Our approach includes:

  • Regular Monitoring: Blood pressure assessments to track and manage hypertension.
  • Lifestyle Guidance: Education on diet and lifestyle modifications to help reduce blood pressure.
  • Comprehensive Support: Ongoing care and advice tailored to each patient’s needs, ensuring optimal management of hypertension.

Continuous Care:

Patients will receive regular follow-ups to adjust their management plans as needed. Those achieving control of their hypertension will be gradually transitioned back to their Primary Care Provider for ongoing maintenance, which includes semi-annual or annual check-ins.

Eligibility and Joining the Program:

Adult patients aged 18 or older with a diagnosis of uncontrolled hypertension or a history of elevated blood pressure are eligible. New patients need a referral from their Primary Care Provider to join and will initially meet with our Family Health Team Nurse. This program is available exclusively to rostered patients of physicians at the Brighton Quinte West Family Health Organization. To join, you will need a referral from your family physician.  Lakeview Family Health Team is committed to providing expert care to help manage your hypertension and enhance your overall heart health.
